Experiencing failure in charged nitrogen pressure vessel.

Original design, 4140 annealed cylinder (threads into plate, seals on oring) with chrome internal seal surface. Only internal seal surface is chrome plated. Other surfaces are painted or bare steel.

New design, 4140 annealed cylinder with Ferritic Nitro Carborizing (FNC). All surfaces are FNC'd.

In last year, experienced 3 cracked cylinders with new design. It cracks at radius above threads behind oring seal.

Only change is listed above. Any ideas what could cause failure?
